Achieving a neutral mechanical axis is paramount for long-term implant survival in orthopedic surgery. The valgus cut angle TKA represents the difference between the anatomical and mechanical axes of the femur. While many surgeons utilize a fixed angle, understanding individual anatomical variations is crucial for precision. Recent research indicates that specific radiological factors can significantly impact the ideal distal femoral cut, especially in diverse populations.
In a detailed study of the Turkish population, researchers identified that the mean valgus cut angle was 6.48 degrees. However, individual values varied significantly based on proximal femoral anatomy. The analysis revealed a moderate negative correlation between the neck-shaft angle and the required cut. Specifically, as the neck-shaft angle decreases, the required cut angle tends to increase. Furthermore, medial offset also plays a role in determining this surgical parameter. Therefore, relying on a standard 5 or 6-degree cut may lead to coronal malalignment in a subset of patients.
Gender does not appear to influence distal femoral anatomy in a statistically significant way. Consequently, preoperative planning should focus more on precise radiological measurements than demographic traits. Utilizing long-leg orthoradiographs allows surgeons to measure the mechanical femorotibial angle and other key variables accurately. This individualized approach ensures that the distal femoral cut remains perpendicular to the mechanical axis. Ultimately, tailoring the procedure to the patient's unique anatomy minimizes the risk of early implant loosening and enhances functional results.
