Identifying subclinical psoriatic arthritis represents a critical opportunity for early intervention. Because this stage occurs before clinical synovitis, it provides a unique window to prevent permanent joint damage. Consequently, the EULAR definition relies on arthralgia or ultrasound evidence of inflammation. Specifically, a recent study from the SPAWCH cohort evaluated 306 patients to characterize this intermediate phase. To conclude, musculoskeletal ultrasound is the dominant feature in the subclinical phase. Because this stage bridges skin psoriasis and active arthritis, timely recognition is paramount. Since early treatment improves outcomes, this screening is vital. Furthermore, by monitoring patients with long-standing psoriasis, clinicians can ensure early referral. Consequently, this approach may significantly alter the disease trajectory. In short, early diagnosis remains the primary goal for preventing disability in psoriasis patients.
