Recent clinical evidence suggests that serum cholinesterase levels serve as a significant predictor for early neurological deterioration (END) in stroke patients. While clinicians traditionally use serum cholinesterase to assess liver function or organophosphate poisoning, its role in acute ischemic stroke is gaining considerable attention. Specifically, in cases of mild anterior circulation stroke, identifying those at risk of sudden worsening is critical for improving long-term outcomes.
A retrospective study involving 334 patients investigated the correlation between admission CHE levels and subsequent clinical decline. Researchers categorized participants based on quintiles of their serum cholinesterase levels and monitored for END within the acute phase. The findings demonstrated a clear linear trend, showing that lower levels were associated with a significantly higher risk of deterioration. Consequently, these results provide a potential new tool for risk stratification in neurology departments.
The study found that the lowest quintile of CHE had an adjusted odds ratio (OR) of 4.14 compared to the highest quintile. This association remained robust even after adjusting for confounding variables such as age, baseline stroke severity, and metabolic factors. Furthermore, low cholinesterase might reflect an underlying systemic inflammatory response or a state of poor nutritional reserve. Therefore, monitoring these levels could help identify high-risk patients who require more intensive surveillance or early intervention.
Moreover, the simplicity of the test makes it a practical addition to existing stroke protocols. Because serum cholinesterase levels are easily measured in standard laboratory panels, implementing this marker does not require specialized equipment. In conclusion, clinicians should consider CHE levels as a part of the comprehensive assessment for patients presenting with mild anterior circulation ischemic stroke.
Lower levels of serum cholinesterase are independently linked to a higher risk of early neurological deterioration. This likely reflects the patient's systemic inflammatory status or nutritional baseline, both of which influence stroke recovery.
Current research specifically highlights its predictive value in mild anterior circulation ischemic strokes. Further studies are needed to determine its utility in more severe or posterior circulation strokes.
Serum cholinesterase is a cost-effective and routinely available marker. Unlike complex biomarkers, it is easily accessible in most clinical settings, facilitating rapid risk assessment.
