Robotic guided implant placement is transforming modern dental surgery by enhancing precision during immediate procedures. A recent systematic review and network meta-analysis highlights that computer-assisted protocols significantly outperform traditional freehand techniques. While all guided methods improve clinical outcomes, robotic-assisted surgery offers the highest level of accuracy in both angular and apical positioning.
Clinical data from eighteen studies show that every guided protocol significantly enhances accuracy compared to freehand techniques. Robotic-guided surgery (rCAIS) provides the most substantial improvement among the evaluated methods. Specifically, mean angular deviation decreased by 3.36° with robotic systems. In contrast, dynamic navigation (dCAIS) showed a 2.66° reduction. Static guides, whether fully or half-guided, also improved precision but to a lesser extent than robotic systems.
Furthermore, robotic systems excel in apex placement. The study recorded a mean apex deviation reduction of 0.90 mm with rCAIS compared to freehand methods. This level of precision is critical for immediate implant placement, where anatomical constraints are often tight. Although platform deviation improved across all guided protocols by approximately 0.6 mm, the robotic approach remains the superior choice for complex apical and angular control.
Surgeons should consider the benefits of these advanced technologies when planning immediate implants. Robotic systems automate portions of the drilling process, which minimizes human error and increases repeatability. Consequently, patients benefit from more predictable results and potentially fewer postoperative complications. However, practitioners must weigh the high cost of robotic equipment against these significant clinical gains. As technology matures, these systems will likely become more accessible to the broader dental community in India and globally.
