Managing residual cement implant crowns remains a significant hurdle in restorative dentistry. Clinical experts recognize excess luting agents as a primary risk factor for peri-implantitis. A recent randomized controlled trial investigated the frequency and quantity of this excess. Specifically, researchers compared resin composite and glass ionomer cements on customized ceramic abutments. They assessed the impact of intraoral exposure times ranging from immediate removal to four weeks post-cementation.
The study involved 40 patients receiving monolithic zirconia crowns. One experienced clinician performed all cementation and excess removal procedures. Surprisingly, the analysis detected residual cement on 97.5% of the abutments. Resin composite cement left significantly more residues than glass ionomer across all measured surfaces. Furthermore, the amount of residue increased significantly when the crowns remained in the mouth for four weeks. However, factors like quadrant location or implant region did not significantly influence the results.
Consequently, the choice of cement material plays a vital role in long-term peri-implant health. Glass ionomer cements appear easier to clean and leave fewer traces behind. Therefore, clinicians must exercise extreme caution when using resin-based materials. In addition, subgingival margins tended to harbor more excess than supragingival locations. These findings highlight the inherent difficulty in achieving a completely clean environment around dental implants.
