This meta-analysis of 6 trials assessed pyridostigmine for orthostatic hypotension (OH). Pyridostigmine alone showed a non-significant reduction in orthostatic BP drop. Combined with midodrine, it significantly improved systolic orthostatic drop. Pyridostigmine minimally affected supine BP and had few side effects. It may be particularly beneficial for patients with preserved baroreflex function and those at risk of supine hypertension.
