Recent clinical evidence suggests a significant link between premature adrenarche and diabetes in youth. While physicians often consider premature adrenarche (PA) a benign variation of normal development, emerging data indicate a complex relationship between adrenal androgen excess and metabolic dysfunction. A recent retrospective study investigated how PA contributes to the development of dysglycemia, including prediabetes (PreDM) and type 2 diabetes (T2DM), in pediatric populations. The researchers found that nearly half of the youth diagnosed with PA eventually faced some form of glucose intolerance.
The study analyzed 229 patients, of whom 82% were already overweight or obese at the time of their PA diagnosis. Among the 90 patients with available follow-up data, the overall frequency of dysglycemia reached 44%. Specifically, 38% of both male and female patients developed prediabetes. Furthermore, 8% of males and 6% of females progressed to type 2 diabetes. These results highlight that PA may serve as an early marker for future metabolic challenges, rather than just a solitary cutaneous or hormonal finding.
Risk factors for progression varied slightly between genders. In females, higher height, weight, and BMI at the initial PA evaluation strongly predicted the later development of prediabetes. However, in males, the researchers noted that those who developed PreDM were more likely to be of Non-Hispanic Black ethnicity. Consequently, pediatricians should maintain a high index of suspicion for metabolic issues when evaluating children with early pubic hair or body odor. Monitoring hemoglobin A1c (HbA1c) levels in these patients appears crucial for early intervention. Because youth-onset T2DM is often more aggressive than adult-onset forms, identifying these high-risk children early could significantly improve long-term health outcomes.
While often benign, premature adrenarche is increasingly linked to obesity, insulin resistance, and future dysglycemia. Regular metabolic monitoring is recommended, especially for children with a high BMI.
Higher BMI, weight, and height at the time of PA diagnosis are significant predictors of future prediabetes, particularly in female patients.
The study suggests a high frequency of dysglycemia (44%), so clinicians should consider periodic HbA1c testing and lifestyle counseling as part of the standard follow-up for these patients.
