Chronic subdural hematoma represents one of the most frequent neurosurgical conditions encountered in aging populations. Because many elderly individuals receive antiplatelet therapy for vascular comorbidities, neurosurgeons face persistent dilemmas regarding surgical timing. Deciding on optimal perioperative aspirin discontinuation requires balancing acute rebleeding complications against catastrophic thromboembolic risks. Recent evidence from a European multicenter cohort offers pivotal clarity on this clinical question.
Chronic subdural hematoma frequently develops following minor trauma in geriatric patients who take daily antiplatelet therapy. Burr-hole craniostomy accompanied by closed-system subdural drainage serves as the standard operative intervention. However, surgeons lack definitive consensus regarding whether to operate immediately or delay intervention to allow platelet recovery. Continuing aspirin may exacerbate acute postoperative hemorrhage within the freshly evacuated subdural cavity. Conversely, prolonging cessation could precipitate myocardial infarction or ischemic stroke in vulnerable individuals with preexisting cardiovascular disease.
Historically, surgical teams adopted wide variations in practice based largely on institutional habit rather than high-level evidence. Some centers advocate for brief interruptions, whereas other teams insist on withholding antiplatelet agents for a week or longer. These clinical discrepancies directly affect hospital throughput, operational planning, and acute neurosurgical outcomes. Therefore, assessing the real-world impact of drug interruption timelines provides essential guidance for clinical decision-making.
To evaluate these therapeutic trade-offs, investigators performed a multicenter retrospective cohort study across four academic European neurosurgical institutions. The authors analyzed 140 consecutive adult patients who received low-dose aspirin and subsequently underwent burr-hole craniotomy with subdural drainage for chronic subdural hematoma. Researchers stratified the cohort into three distinct management arms based on drug cessation timing. The continuation group comprised 24 patients who maintained aspirin throughout the surgical window.
The short-pause cohort included 47 patients whose aspirin was held for five days or fewer prior to surgery. In contrast, the long-pause cohort consisted of 69 individuals who discontinued therapy more than five days before craniotomy. Investigators examined primary endpoints consisting of early postoperative hemorrhagic events within 48 hours and thromboembolic events occurring within 30 days. Secondary measures tracked the Glasgow Outcome Scale at discharge and one month, the modified Rankin Scale at six months, length of hospital stay, 90-day recurrence, reoperation, and overall mortality.
The study demonstrated clear differences in early bleeding complications among the stratified groups. Overall, six acute hemorrhagic events occurred within 48 hours among 104 patients evaluated at one month. Specifically, four out of 19 patients in the continuation group experienced acute postoperative hematomas, representing a 21.1% complication rate. In sharp contrast, zero patients in the short-pause cohort and only two patients in the long-pause cohort experienced acute bleeding.
When pooling all discontinued patients, the bleeding incidence was merely 2.4% compared to 21.1% in the continuation arm. Multivariable logistic regression revealed that continuing aspirin independently increased the odds of acute postoperative hemorrhage by more than eleven-fold after adjusting for age, Charlson Comorbidity Index, baseline functional status, and prophylactic indications. Furthermore, thromboembolic events remained remarkably rare across the entire cohort. Only one single patient developed a thromboembolic event, and that individual belonged to the continuation group. Consequently, brief discontinuation did not trigger an excess of ischemic complications.
Beyond immediate hemostatic safety, perioperative antiplatelet management significantly influenced patient neurological recovery. Patients managed with aspirin cessation achieved superior functional scores at one-month follow-up on the Glasgow Outcome Scale. Moreover, this functional advantage persisted at six months, where patients in the discontinuation groups demonstrated significantly better modified Rankin Scale scores. Preventing acute rebleeding appears critical for averting secondary cortical injury and functional decline in older patients.
Importantly, the timing of drug cessation did not alter primary long-term neurosurgical metrics. The overall three-month recurrence rate was 7.7%, showing no significant statistical differences between continuation, short-pause, and long-pause groups. Similarly, rates of surgical revision, overall length of hospital stay, and one-month mortality remained equivalent across cohorts. These results indicate that short-term drug withdrawal safely mitigates acute hemorrhagic risks without compromising procedural success or extending hospitalization.
The authors noted that baseline clinical characteristics were broadly comparable, with one notable exception involving hemostatic reversal agents. Clinicians administered perioperative desmopressin much more frequently to patients in the continuation group to counteract antiplatelet effects. Specifically, 54.2% of continuing patients received desmopressin, compared to 8.5% in the short-pause arm and 1.5% in the long-pause arm. Nevertheless, despite higher utilization of desmopressin, patients continuing aspirin still suffered significantly elevated postoperative hemorrhagic rates.
To exclude confounding from rescue medications, investigators performed a sensitivity analysis excluding all individuals who received desmopressin. This sub-analysis confirmed consistent findings, with persistent bleeding in 22.2% of continuing patients versus 2.5% of discontinued patients. Therefore, empirical administration of desmopressin failed to neutralize the hemorrhage risk associated with active aspirin therapy during burr-hole drainage. Surgeons cannot rely solely on pharmacological reversal to bypass the necessity of temporary drug interruption.
In Indian clinical settings, neurosurgeons and neurocritical care teams frequently treat elderly patients presenting with chronic subdural hematoma against a backdrop of coronary artery disease and metabolic syndrome. Clinicians often struggle when determining how long to delay urgent decompression. This multicenter European investigation delivers two practical insights for neurosurgical teams. First, active aspirin maintenance exposes patients to substantial acute rebleeding risks that pharmacological reversal cannot reliably prevent.
Second, extending the preoperative drug suspension beyond five days yields no demonstrable clinical benefit over a shorter pause of five days or fewer. Unnecessarily prolonged delays can increase in-hospital immobility, pulmonary complications, and caregiver burden in resource-constrained environments. Consequently, pursuing an individualized, short pause of up to five days optimizes hemostatic control while maintaining cardiovascular safety. Collaborative dialogue between neurosurgeons, cardiologists, and anesthesiologists ensures timely operative intervention, improved functional independence, and minimal surgical morbidity.
In this multicenter cohort, temporary aspirin discontinuation did not increase thromboembolic complications. Only one patient in the entire study experienced a thromboembolic complication, and that individual belonged to the continuation group. Therefore, short pauses do not appear to trigger an excess of acute arterial or venous thrombotic events.
The study demonstrated that pausing aspirin for five days or fewer achieved identical hemostatic safety compared to pausing for more than five days. Because both cessation arms had low bleeding rates without differences in recurrence, a short pause of up to five days provides effective protection without unnecessary surgical delays.
Aspirin continuation did not reduce or increase the three-month recurrence rate after burr-hole craniostomy with subdural drainage. Overall recurrence remained low at 7.7% across all patients. However, continuing aspirin dramatically elevated acute 48-hour postoperative bleeding risks, which ultimately impaired six-month functional recovery scores.
