Managing moderate-severe pediatric systemic lupus erythematosus therapy often presents significant clinical challenges. Specifically, clinicians must balance aggressive disease control with the risks of long-term steroid toxicity. A recent cohort study explored the systematic administration of rituximab (RTX) combined with cyclophosphamide (CYC) as a potential solution for these patients.
The study followed eleven patients over a ten-year period from 2013 to 2023. Notably, the cohort was racially diverse, with over 50% identifying as Black. The researchers focused on primary outcomes such as Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) scores and steroid exposure. In conclusion, this research supports the use of combined rituximab and cyclophosphamide for pediatric cases. It demonstrates high efficacy across diverse racial groups. However, larger prospective trials are necessary to refine these protocols further.
