Children with brain arteriovenous malformations (bAVMs) often face significant neurological challenges that extend beyond physical symptoms. While clinicians traditionally focus on hemorrhage risks, these vascular anomalies can severely disrupt neurodevelopment. A recent cross-sectional study has shed light on the Pediatric Brain AVM Neuropsychology, revealing that cognitive deficits are surprisingly prevalent even in unruptured cases.
The study analyzed 17 patients aged 6 to 16 years who were treated between 2016 and 2023. Researchers utilized the Brief Neuropsychological Assessment Battery to evaluate various domains, including attention, working memory, and language. Surprisingly, the results indicated that nearly 90% of the participants exhibited some form of cognitive impairment. Specifically, working memory and executive functions were the most frequently affected areas.
The investigation aimed to find links between the physical characteristics of the malformation and cognitive performance. Interestingly, researchers discovered that lesions located in non-eloquent areas were associated with lower scores in the memory domain. However, other factors like lesion size, lobe localization, or Spetzler-Martin grade did not show significant associations with specific deficits. This finding suggests that the impact of a bAVM on the developing brain is complex and not always predictable by standard surgical grading systems.
Furthermore, the high incidence of impairments in both ruptured and unruptured patients highlights a critical need for early intervention. Because these deficits interfere with social adaptation and academic success, a comprehensive evaluation is necessary for every patient. Consequently, healthcare providers in India and globally should integrate neuropsychological testing into the standard care protocol for pediatric vascular anomalies. This approach ensures that we address the long-term quality of life alongside surgical or radiological treatments.
