In the field of joint preservation, surgeons debate the efficacy of medial meniscus root repair during corrective osteotomy. A recent randomized trial investigated this specific issue. It compared open-wedge high tibial osteotomy (OWHTO) with and without concurrent arthroscopic centralization. The study aimed to see if adding these procedures improves clinical or radiological outcomes for medial meniscus posterior root tears.
The trial enrolled 82 patients between 50 and 75 years of age. These patients had symptomatic tears and mild-to-moderate varus alignment. Researchers divided participants into an isolated OWHTO group and a combined treatment group. Surprisingly, the final results showed no significant differences in clinical outcome scores. Both groups experienced similar reductions in pain. Furthermore, their functional scores improved at comparable rates. Therefore, adding complex meniscal procedures did not provide superior short-term results for these patients.
Additionally, radiological assessments showed no major benefits from the combined approach. Medial meniscus extrusion levels remained similar across both groups after surgery. Second-look arthroscopy also confirmed that healing rates were comparable. Consequently, the researchers concluded that the mechanical unloading from OWHTO is likely the primary success factor. While centralization aims to stabilize the joint, it may not be necessary in every case.
Evidence suggests that adding centralization to OWHTO does not significantly improve clinical outcomes for medial meniscus root repair in patients with varus malalignment.
OWHTO primarily unloads the medial compartment. This shift reduces pressure on the damaged meniscus. As a result, patients experience pain relief and improved knee function regardless of the repair technique used.
