Radiation oncologists are currently refining oropharyngeal cancer radiation therapy to capitalize on the favorable prognosis of HPV-mediated (HPV+) disease. While definitive treatment doses remain standard, a recent survey reveals striking differences in how specialists manage Clinical Target Volume (CTV) expansions and postoperative dosing. Consequently, understanding these variations is crucial for designing future clinical trials that aim to balance cure with quality of life.
A new survey of 55 expert head and neck radiation oncologists showed that nearly all physicians maintain definitive doses of approximately 70 Gy for both HPV-positive and HPV-negative cancers. However, significant practice variations exist regarding the CTV expansion. Although the average expansion is 3.6 mm, nearly half of the respondents use smaller margins of 0 to 3 mm for HPV-positive cases. Additionally, while most practitioners use identical expansions for lymph nodes across both disease types, a notable minority applies smaller margins specifically for HPV-positive nodes. These findings suggest that experts are increasingly tailoring their approach based on viral status.
Discrepancies are even more pronounced in the postoperative setting. For HPV-negative disease, 96% of radiation oncologists prescribe 60 Gy. In contrast, for HPV-positive cases, only 52% treat to 60 Gy, while 35% have already adopted a de-escalated dose of 50 Gy. Therefore, many clinicians are proactively reducing treatment intensity for HPV-positive patients despite the lack of a universal consensus. Furthermore, the use of smaller target volumes indicates a growing confidence in the localized control of these specific tumors. In contrast, standardizing these margins remains a challenge for the oncology community.
Consequently, these results highlight the necessity for standardized guidelines to reduce treatment heterogeneity across institutions. While experts are moving toward de-escalation, the variation in margins and doses emphasizes the need for definitive trial data to guide best practices. Future studies should focus on establishing clear thresholds for CTV margins to ensure consistent patient outcomes.
