The National Institute for Occupational Safety and Health (NIOSH) recently completed a comprehensive multi-site exposure assessment among forensic laboratory personnel. This investigation highlights critical concerns regarding occupational fentanyl exposure and other illicit substances. Forensic scientists routinely handle unknown evidence that requires weighing and chemical processing. Consequently, they face potential risks from inhalation, ingestion, and accidental dermal contact.
NIOSH researchers evaluated several exposure pathways by collecting personal air samples, hand wipes, and surface samples across eleven sites. Additionally, they performed personal biomonitoring through urine samples to check for cocaine, fentanyl, heroin, and methamphetamine. The findings were revealing. Notably, thirteen personal air samples exceeded the American Conference of Governmental Industrial Hygienists (ACGIH) Threshold Limit Value (TLV) for fentanyl. This limit is currently set at 100 ng/m³ for an eight-hour time-weighted average.
Contamination was not limited to the air. The study found that surface wipes exceeded the ACGIH Surface Limit at multiple facilities. Researchers identified detectable concentrations on several high-touch surfaces. These included laboratory scales, keyboards, return air vents, and even the tops of light fixtures. Moreover, eight out of eleven sites had at least one staff member with detectable drug metabolites in their urine. This confirms that systemic absorption can occur despite current laboratory controls.
To minimize these risks, laboratories must adopt best practices from established scientific literature. Since the long-term health effects of chronic low-dose exposures are not fully understood, prudent safety calls for minimizing all contact. Specifically, lab managers should optimize ventilation and ensure the consistent use of personal protective equipment. Furthermore, regular decontamination of shared equipment remains a vital step in maintaining a safe environment.
The primary routes include the inhalation of aerosolized powders during evidence handling and dermal contact with contaminated surfaces. This study highlights that contamination on keyboards and scales is a common source of transfer.
While fentanyl has established ACGIH limits, many other drugs like methamphetamine or heroin currently lack standardized occupational thresholds. Therefore, workers must follow the principle of keeping exposures as low as reasonably achievable (ALARA).
NIOSH recommends using high-performance laboratory hoods and ensuring building ventilation meets ANSI/ASSP Z9.5 standards. These controls are essential to prevent airborne particulates from entering the general breathing zone of lab personnel.
