India continues to battle a high tuberculosis burden according to the Global TB Report 2025. Consequently, Molecular TB Diagnostics have emerged as the new standard of care across the country. These tests offer far greater accuracy than traditional smear microscopy. Smear microscopy often missed one in two cases due to its low sensitivity. Therefore, rapid molecular platforms are now essential for early and accurate detection.

Molecular TB Diagnostics like Truenat and GeneXpert provide rapid and automated results. They detect Mycobacterium tuberculosis and identify resistance to rifampicin within two hours. India’s National Tuberculosis Elimination Programme now utilizes these tools as frontline tests. Specifically, the Truenat platform serves as a point-of-care solution in primary health centers. Furthermore, these tests are cost-effective at government facilities, costing between Rs 600 and Rs 800. Currently, about 45% of TB centers use these upfront molecular tests. Traditional diagnosis typically requires a sputum sample for testing. Unfortunately, many patients, especially children and the elderly, struggle to produce sputum. New innovations from Indian firms like Molbio Diagnostics solve this issue. They utilize tongue swabs for easier sample collection while maintaining high accuracy. The World Health Organization recently recommended these near-point-of-care tests for pilot introduction. Additionally, ultra-portable digital X-rays help identify asymptomatic carriers who may not visit clinics. Artificial intelligence screens these X-rays to find potential cases instantly. Then, molecular tests confirm the final diagnosis. This combined approach significantly strengthens India’s goal to eliminate tuberculosis.
