Managing patients with refractory hyperprolactinemia and multiple sellar masses requires high diagnostic precision. Consequently, when MRI findings show more than one lesion, clinicians face a significant challenge in surgical planning. In these complex scenarios, IPSS for prolactinoma localization provides essential functional data. Moreover, by sampling prolactin levels directly from the venous drainage, surgeons can identify the secreting adenoma with confidence. Therefore, this procedure bridges the gap between anatomical imaging and functional pathology.
Traditionally, inferior petrosal sinus sampling (IPSS) differentiates Cushing disease from ectopic ACTH production. However, this technique also serves as a powerful diagnostic tool for localizing prolactinomas. For instance, in a notable illustrative case, a 37-year-old female presented with two distinct pituitary masses on MRI. Because dopaminergic therapy failed to control her symptoms, surgery became necessary. To avoid unnecessary bilateral exploration, clinicians performed IPSS to measure prolactin and ACTH levels.
Furthermore, they assessed lateralization using both absolute prolactin levels and values normalized to ACTH. Notably, this normalization process accounts for variations in venous drainage, ensuring more accurate results. Consequently, the data clearly identified the right-sided lesion as the source of hypersecretion. Following this guidance, the surgical team performed a targeted endonasal endoscopic resection. As a result, the patient achieved complete remission for over two years without any signs of hypopituitarism.
Thus, this procedure proves highly beneficial for patients with bilateral pituitary tumors. In addition, it helps surgeons minimize tissue removal while maximizing therapeutic success. Moreover, IPSS reduces the risk of post-surgical complications like hormone deficiencies. Therefore, physicians should consider this approach when imaging alone cannot pinpoint the source of refractory hormone excess.
Normalization to ACTH levels corrects for potential asymmetries in the venous anatomy. This step ensures that a higher prolactin level truly represents localized secretion rather than simply a more dominant drainage pathway on one side.
While IPSS is an invasive procedure requiring skilled interventional radiologists, it is generally safe. The risk of major complications remains low. Most importantly, it provides critical information that prevents larger, more invasive bilateral surgeries.
Physicians should consider this technique for patients with symptomatic, refractory hyperprolactinemia who have multiple lesions on MRI. It is particularly useful when surgical resection is planned and the surgeon must target a specific mass.
