Carotid web thrombus visualization represents a significant hurdle in the management of cryptogenic strokes, particularly in younger patients. These non-atheromatous shelf-like projections in the internal carotid artery often escape detection on standard imaging. Furthermore, the natural history of untreated carotid webs involves alarmingly high rates of recurrent ischemic events. Consequently, clinicians must maintain a high index of suspicion when evaluating patients with unexplained hemispheric strokes. This illustrative case underscores the limitations of preoperative angiography and the transformative role of real-time imaging.
A 44-year-old man presented with four recurrent right hemispheric ischemic events over five years. Despite multiple previous angiographic studies, clinicians initially misinterpreted the underlying pathology. Eventually, digital subtraction angiography revealed a subtle posterolateral carotid web. Therefore, the surgical team scheduled a carotid endarterectomy for definitive treatment. However, the most critical finding occurred just before the first incision. Using intraoperative ultrasound, the surgeons achieved clear carotid web thrombus visualization, identifying a large adherent clot that preoperative imaging had completely missed.
The discovery of the dynamic thrombus immediately influenced the surgical approach. Surgeons performed a successful en bloc removal of both the web and the adherent thrombus. Histopathological analysis subsequently confirmed the diagnosis of an intimal variant of fibromuscular dysplasia. Notably, the patient remained entirely stroke-free at his one-year follow-up examination. This outcome suggests that surgical intervention, guided by real-time vascular imaging, provides excellent long-term results for symptomatic patients.
Atypical carotid webs often lack classic radiographic signatures and can be misclassified on noninvasive scans. Moreover, web-associated thrombi are highly dynamic. This means they may not appear on high-resolution angiography performed even shortly before surgery. Intraoperative ultrasound provides a real-time assessment of the thrombus burden. Specifically, it allows for improved surgical planning and ensures that the entire embolic source is removed. As a result, this modality should be considered a standard adjunct in the surgical management of symptomatic carotid webs.
A carotid web is a thin, shelf-like projection of intimal tissue that protrudes into the lumen of the carotid artery, often causing blood flow stagnation and thrombus formation.
Angiography may miss these thrombi because they are often dynamic or positioned in ways that standard 2D views do not clearly capture. Real-time ultrasound offers a more detailed, 3D-like assessment of the vessel wall.
While medical therapy is common, surgical endarterectomy or carotid stenting are often preferred for symptomatic cases due to the high risk of recurrent stroke with antiplatelet therapy alone.
