Managing diabetes in a hospital setting requires a delicate balance between standardized clinical protocols and individual patient expertise. While hospital guidelines are designed for safety, many patients find that inpatient diabetes self-management is often compromised during their stay. Consequently, this disconnect can lead to avoidable harms and suboptimal glycemic control.
A recent qualitative study explored the experiences of adults with type 1 and type 2 diabetes who were hospitalised for various medical reasons. The findings revealed that the inpatient environment frequently limits self-management behaviours. Moreover, these restrictions apply even to patients who are both willing and capable of managing their own care. Therefore, understanding these barriers is essential for improving hospital protocols and patient safety.
The research identified three primary ways in which self-management is discouraged. First, healthcare teams often fail to account for a patient\'s existing knowledge and willingness to self-manage upon admission. Furthermore, patients often feel that their involvement in decisions regarding ongoing diabetes care is severely limited. Finally, the inherent lack of flexibility in hospital schedules prevents patients from adhering to their established management routines.
These systemic restrictions may contribute to less effective diabetes management and poorer clinical outcomes. For instance, when hospital meal times and medication schedules do not align with a patient\'s needs, the risk of glucose fluctuations increases. In addition, ignoring patient expertise can lead to emotional distress and a lack of trust in the medical team. Consequently, clinicians should strive to incorporate patient involvement into inpatient care plans to enhance safety.
To reduce harm, hospital practices must become more conducive to self-management. Since patients are the primary managers of their condition at home, their insights are invaluable in the clinical setting. Incorporating flexibility into hospital schedules and respecting patient autonomy can lead to more stable glycemic levels. Therefore, future work must focus on integrating patient involvement into the standard of care for all hospitalised individuals with diabetes.
