Recent research highlights significant fungal disease diagnostic gaps that continue to hinder public health efforts in resource-limited settings. A nationwide survey conducted in Peru investigated the laboratory capacity of 42 hospitals, revealing substantial deficiencies in infrastructure and technology. These findings are particularly relevant to medical professionals in India, as both regions face a high burden of invasive fungal infections combined with limited diagnostic resources. Consequently, clinicians often struggle to identify life-threatening pathogens in a timely manner.
The study found that only 26% of surveyed laboratories had dedicated mycology areas. Furthermore, over half of these facilities reported having less than 20 square meters of workspace. While microscopy remains widely available, more advanced molecular tools are strikingly scarce. For instance, PCR was utilized in less than 5% of laboratories, and no facility performed DNA sequencing. Most mold identification still relies almost entirely on traditional morphology, which can delay definitive diagnosis. Additionally, regional disparities were evident, with hospitals in Lima possessing better equipment and more staff than those in other regions.
To improve patient outcomes, health systems must address the inconsistent access to antifungal susceptibility testing (AST) and essential medications. The survey indicated that AST was available in only 43% of laboratories, primarily for yeast species. Moreover, access to key antifungal drugs remains uneven, often favoring urban centers. This disparity creates a significant barrier to effective treatment for patients in rural or underserved areas. Therefore, strengthening laboratory infrastructure and ensuring the equitable distribution of medicines are vital steps for any national health strategy.
Furthermore, the survey revealed a lack of standardized training and quality assurance. Only 36% of the laboratories participated in external quality assessments, and just 17% reported active mycology training programs. These statistics suggest that even when equipment is present, the expertise required to interpret complex results may be lacking. Strengthening mycology services through continuous education and robust quality control is essential for modernizing fungal disease management.
The main barriers include a lack of molecular diagnostic tools like PCR, insufficient laboratory workspace, and a shortage of trained mycologists. These factors often lead to reliance on slower, less sensitive traditional methods.
AST is critical because it helps clinicians select the most effective treatment and monitors for emerging drug resistance. Limited access to AST can lead to suboptimal therapy and increased mortality rates.
Regional disparities often result in a concentration of advanced technology and specialists in capital cities. This leave patients in outer regions with delayed diagnoses and reduced access to life-saving antifungal medications.
