Clinicians managing diabetic retinopathy often face challenges with patients who exhibit a suboptimal response to standard anti-VEGF therapies. However, a recent study in Singapore provides compelling evidence. This research shows that switching to Faricimab for chronic DME significantly improves both functional and anatomical outcomes. Researchers analyzed a large cohort of patients who previously underwent extensive therapy but still required more effective management for persistent macular edema.
In addition, the retrospective study included 157 eyes from 131 patients who transitioned to faricimab between 2022 and 2024. Most of these patients had received prior anti-VEGF treatments for a median of 1.8 years. After the loading phase, the mean best available visual acuity (BAVA) increased from 57.8 to 62.4 ETDRS letters. Specifically, this significant improvement suggests that the dual-pathway inhibition offered by faricimab effectively restores visual function in chronic cases. Faricimab targets both VEGF-A and Ang-2 to stabilize the retinal vasculature.
Notably, the study observed remarkable anatomical changes. The mean central subfield thickness (CST) dropped from 398.8 µm at baseline to 318.0 µm at the final follow-up. This reduction indicates a robust drying effect on the macula. It works even in eyes that were previously resistant to other agents. Furthermore, the researchers noted that 15.3% of the eyes achieved complete resolution of intraretinal or subretinal fluid.
A key aspect of this research was the comparison between different loading regimens. Clinicians administered either three or four consecutive injections based on their professional judgment. In fact, the subgroup analysis revealed no significant difference in outcomes between these groups. Consequently, both cohorts experienced substantial gains in visual acuity. They also saw significant reductions in retinal thickness. This finding suggests that a personalized approach to the loading phase can be effective without compromising patient outcomes.
Moreover, the safety profile remained favorable throughout the study period. No new safety concerns or cases of intraocular inflammation were reported. This reliability is crucial for practitioners in India. Long-term management of diabetic complications requires both efficacy and safety. By addressing the Ang-2 pathway alongside VEGF, faricimab stabilizes the vasculature. It also reduces the overall treatment burden for many patients.
Faricimab is a bispecific antibody that inhibits both Vascular Endothelial Growth Factor (VEGF-A) and Angiopoietin-2 (Ang-2). While standard agents only target VEGF, Faricimab’s dual inhibition helps stabilize the retinal vasculature more effectively. This potentially improves outcomes in treatment-resistant cases.
While standard protocols often suggest a loading phase, this real-world study found that three consecutive injections yielded similar results. Both three and four injections provided improvements in patients switching from other therapies. Treatment should always be tailored to the individual patient’s clinical response.
The study reported a significant reduction in central subfield thickness (CST) and the resolution of retinal fluid. Approximately 15% of eyes achieved complete dryness. There was a mean CST reduction of nearly 80 µm after the switch.
