Chronic disease management in India is undergoing a digital transformation. Consequently, understanding the determinants of digital health intervention usage is becoming vital for clinicians. Digital health interventions (DHIs) offer a promising way to enhance self-management in patients with type 2 diabetes (T2DM) and coronary heart disease (CHD). However, the success of these tools depends heavily on whether patients actually use them. Recent research has highlighted that the factors driving initial uptake differ from those that sustain frequent use.
The initial decision to engage with a digital health portal is often influenced by sociodemographic and psychological traits. Research indicates that higher levels of education significantly increase the likelihood of a patient trying a DHI for the first time. Furthermore, personality traits such as openness to new experiences play a crucial role. Patients who possess a proactive mindset regarding their health, specifically those with strong intentions for physical activity and healthy nutrition, are more likely to initiate use. Interestingly, while education and personality drive the first click, they do not necessarily guarantee continued engagement.
Once a patient begins using a web portal, the drivers for frequent engagement shift. For patients with T2DM or CHD, frequency of use is primarily predicted by behavioral intentions and planning. Specifically, having a clear plan for physical activity and nutrition correlates with higher usage rates. In contrast, sociodemographic factors like age and gender do not appear to impact how often a patient returns to the portal. This suggests that while we must target educational barriers to get patients started, we must focus on behavioral planning to keep them engaged over time.
In the Indian clinical context, where patient loads are high, identifying which patients will benefit most from digital tools is essential. Clinicians should assess a patient’s health literacy and readiness for change before recommending a web-based portal. Patients with higher educational backgrounds and clear lifestyle goals are the ideal candidates for initial adoption. However, for long-term adherence, physicians should provide structured support to help patients integrate these digital tools into their daily routines. Focusing on goal setting and action planning can significantly improve the efficacy of these interventions.
Higher education levels often correlate with higher digital literacy. This makes patients more comfortable with initial uptake. However, clinicians must ensure that digital tools are designed inclusively to prevent an increase in health disparities among less-educated populations.
