Effective diabetic macular edema treatment remains a cornerstone in preventing vision loss for patients with diabetes. While intravitreal steroids like dexamethasone implants provide significant relief, clinicians often seek ways to extend treatment efficacy and reduce the injection burden. A recent study has demonstrated that combining these implants with navigated subthreshold yellow (577 nm) micropulse laser (SMLT) offers superior results compared to steroid monotherapy alone.
The prospective randomized study included 80 eyes with center-involving DME. One group received a dexamethasone implant followed four weeks later by navigated SMLT, while the control group received the implant alone. Consequently, at the 6-month mark, the combination group showed a significantly greater improvement in visual acuity, reaching 6.2 letters compared to 4.1 letters in the monotherapy group. Furthermore, this visual gain persisted through the 12-month follow-up, highlighting the long-term benefits of the dual approach.
In addition to visual improvements, the anatomical outcomes were notably better in patients receiving combination therapy. The mean central subfield thickness (CST) was significantly lower in the combination group at both 6 and 12 months. Moreover, the study reported a longer retreatment-free survival period, with a median of 11.2 months for the combination group compared to 8.3 months for monotherapy. Therefore, integrating SMLT may serve as a potent adjunct to stabilize the retinal structure more effectively.
Importantly, the addition of navigated SMLT did not result in any extra safety concerns or adverse events during the trial. This makes it a viable option for reducing the frequency of invasive intravitreal injections. Similarly, previous research in India has suggested that subthreshold lasers can successfully modulate treatment algorithms, potentially reducing the overall healthcare burden. For clinicians, these findings support a proactive transition toward multimodal therapy to optimize patient quality of life.
The 577 nm yellow laser is highly absorbed by oxyhemoglobin and has minimal absorption by foveal pigments like lutein. This allows for precise treatment closer to the fovea with a high safety profile and minimal risk of thermal damage to the neurosensory retina.
Adding navigated SMLT significantly extends the retreatment-free interval. In the study, patients receiving the combination therapy had a median survival of 11.2 months without needing another injection, compared to only 8.3 months for those on monotherapy.
