This network meta-analysis compared pharmacological and biological treatments for ALS using 109 randomized trials involving 16,353 participants. The primary outcome was change in the ALS Functional Rating Scale–Revised (ALSFRS-R), with respiratory function, mortality and serious adverse events also assessed. Compared with placebo, the combination of cell therapy and neuroprotective agents showed the greatest estimated attenuation of functional decline, with a mean difference of 3.65 points (95% credible interval, 1.27–6.05). Receptor agonists ranked highest for preserving forced vital capacity, while alkaloids ranked highest for estimated mortality reduction. However, no intervention demonstrated a statistically significant survival benefit compared with placebo. The authors highlighted substantial heterogeneity across studies, particularly for some drug classes, and cautioned that treatment rankings should not be interpreted as definitive head-to-head evidence. The analysis suggests that combined biological and neuroprotective strategies warrant further investigation, while comprehensive supportive care remains essential.
