Hearing loss significantly impacts individuals in low- and middle-income countries (LMICs). While surgical techniques advance, clinicians must carefully manage cochlear implant surgery complications to protect patient health. Social and structural factors often heighten these risks in resource-constrained environments. Consequently, understanding specific complication profiles is essential for surgical teams globally.
A recent systematic review of thirty studies reveals that complication rates differ by national income level. Specifically, lower-middle-income countries (L-MICs) experience a pooled average rate of 8.83%. In contrast, upper-middle-income countries (U-MICs) maintain a lower rate of 5.08%. Furthermore, the nature of these complications varies significantly between these classifications.
Facial nerve injuries appear more frequently in L-MICs than in other regions. Conversely, U-MICs report higher instances of vertigo, dizziness, and device failure. These differences likely reflect variations in surgical training, technological access, and perioperative infrastructure. Therefore, specialized expertise remains the primary facilitator of safe surgery.
Systemic and procedural issues often create hurdles for safe implantation. For instance, limited capacity and technological gaps can delay necessary interventions. However, multidisciplinary teams and meticulous perioperative planning can mitigate these barriers effectively. Moreover, widening access initiatives help ensure that safe surgery reaches more patients in need.
Overall complication rates in LMICs are generally comparable to those in high-income countries. However, limited follow-up data in some regions may lead to underreporting of late-stage issues.
