Clinical trial registration is a cornerstone of transparent research. It significantly reduces reporting bias and protects the integrity of medical evidence. However, current ICMJE clinical trial registration requirements contain significant ambiguities. Specifically, the recommendations currently exempt studies that evaluate interventions only on healthcare provider (HCP) knowledge or attitudes. This gap creates notable risks for the integrity of the medical literature.
Furthermore, HCP-focused trials are prone to the same biases as patient-centered research. If researchers do not register a trial prospectively, negative results might never reach the public domain. Consequently, this selective reporting can distort the evidence used by policymakers. Experts argue that mandatory registration for all HCP-focused trials is a necessary step to uphold scientific standards. Moreover, healthcare providers who participate in research deserve the same ethical protections as patients. Therefore, registration ensures that their contributions are part of an accountable and transparent process.
Updating these guidelines would also bring the ICMJE in line with other international health research bodies. Many global organizations do not offer exemptions based on the participant type. Additionally, requiring registration for all interventional studies simplifies the regulatory landscape for clinical researchers. Ultimately, this change would foster a more consistent approach to research ethics worldwide. It is time for the ICMJE to revise its recommendations to include all trials involving healthcare professionals.
