Managing patients co-prescribed benzodiazepines and opioids presents significant clinical challenges in primary care. A recent pilot study highlights the value of benzodiazepine tapering support through cognitive-behavioral therapy (CBT). Researchers evaluated whether adding CBT to a standard taper protocol could improve outcomes for patients facing the risks of high-risk medication co-use.
The trial compared a CBT-enhanced taper against a standard health education control. Participants rated both interventions as logical and useful. Although treatment engagement remained similar across groups, the results were promising. Approximately 60% of patients in both conditions achieved at least a 50% reduction in their benzodiazepine dose by the two-month follow-up. Consequently, this reduction significantly lowers the risk of adverse drug-related outcomes and overdose.
While the study did not find statistically significant differences between groups for dose reduction, effect sizes favored the CBT group regarding psychological symptoms. Specifically, patients receiving CBT showed better outcomes for anxiety severity and anxiety sensitivity. Therefore, addressing psychological barriers appears crucial during the tapering process. This behavioral adjunct helps patients manage the distress that often triggers a return to higher drug doses.
Implementing behavioral interventions alongside pharmacological tapering addresses the psychological hurdles of medication cessation. Because high anxiety sensitivity often hinders tapering efforts, CBT provides patients with necessary coping mechanisms. Clinicians should consider psychological adjuncts to improve safety in patients using high-risk drug combinations. By integrating these strategies, primary care providers can better manage the dual risks associated with opioid and benzodiazepine co-use.
