This phase 2 randomized, double-blind, placebo-controlled trial evaluated vemircopan, an oral selective factor D inhibitor that blocks alternative-pathway complement activation, in adults with acetylcholine receptor antibody-positive generalized myasthenia gravis. The study was conducted at 60 sites across eight countries. Seventy participants were randomized to vemircopan 180 mg twice daily (n = 28), vemircopan 120 mg twice daily (n = 14), or placebo (n = 28), with an eight-week primary evaluation period. The primary endpoint was a reduction of at least two points in the Myasthenia Gravis Activities of Daily Living (MG-ADL) score sustained for four consecutive weeks without rescue therapy. The endpoint was achieved by 16/28 participants (57%) receiving 180 mg, 8/14 (57%) receiving 120 mg, and 18/28 (64%) receiving placebo. Neither dose showed a statistically significant benefit over placebo, and no significant differences were observed in secondary efficacy outcomes. No meningococcal infections were reported; however, one participant died from hepatic failure and another discontinued treatment because of herpes simplex meningitis. The study was terminated because the prespecified minimum efficacy threshold was not met. Conclusion: Vemircopan did not demonstrate sufficient efficacy in this trial to support further development under the study’s original criteria. The results highlight the need for additional research before alternative-pathway complement inhibition can be considered an effective treatment strategy for this form of myasthenia gravis.
