Managing refractory bladder pain syndrome (BPS) remains a significant challenge for urologists and pain specialists. While botulinum toxin A (BoNT-A) injections offer relief, the duration of effect often limits long-term success. A new study suggests that bladder thermal irrigation significantly enhances the therapeutic outcomes of these injections for patients who have failed conventional treatments.
Researchers conducted a retrospective cohort study to evaluate whether adjuvant thermal therapy improves the efficacy of ultrasound-guided BoNT-A. Specifically, the team compared two groups based on irrigation temperature. One group received warm saline irrigation at 43°C (TI Group), while the control group received room-temperature saline (CON Group).
The study results revealed a marked improvement in the duration of symptom relief for the TI Group. Notably, the median duration of efficacy reached 34.5 weeks in the bladder thermal irrigation group, compared to only 24 weeks in the control group. Furthermore, at the three-month follow-up, the thermal irrigation group demonstrated significantly greater pain reduction on the Visual Analogue Scale (VAS).
In addition to prolonged relief, the overall treatment success rate was substantially higher with thermal adjuncts. At six months, 71.9% of the TI group reported a successful response, compared to 50% in the control group. Consequently, this simple adjuvant technique might offer a more sustainable solution for patients suffering from severe bladder pain.
Safety remains a paramount concern in intravesical therapies. This study reported a favorable safety profile for the 43°C irrigation, with no increased risk of adverse events. Therefore, clinicians might consider this method to improve the pharmacological uptake of BoNT-A and optimize patient outcomes in refractory cases.
