A decade-long retrospective study has revealed critical shifts in the spectrum of biopsy-proven kidney diseases in Northwest China. Researchers analyzed data from 3,514 native kidney biopsies performed between 2014 and 2023. This analysis helps clinicians understand how disease prevalence changes over time and how these patterns impact management strategies. These findings are particularly relevant for nephrologists who manage complex glomerulonephritis cases in diverse populations.
Primary glomerulonephritis (PGN) remained the most common pathology, accounting for over 72% of total cases. Within this category, membranous nephropathy (MN) and IgA nephropathy (IgAN) were the predominant diagnoses. Interestingly, the proportion of patients with MN and IgAN increased significantly during the latter five years of the study. In contrast, the frequency of focal segmental glomerular sclerosis showed a noticeable decline compared to the earlier period.
Furthermore, secondary glomerulonephritis (SGN) comprised approximately 21% of the biopsy-proven cases. Diabetic kidney disease (DKD) was the leading cause within the SGN group. This rising trend reflects the global surge in metabolic syndromes and diabetes. Conversely, the prevalence of lupus nephritis decreased during the study period. Tubulointerstitial diseases were less common, representing under 3% of the total cohort.
Moreover, clinical indications for biopsy have shifted. Nephrotic syndrome remains a major trigger for biopsy. However, clinicians are now identifying more cases through early screening for asymptomatic proteinuria. Consequently, early intervention remains a cornerstone for preventing progression to end-stage renal disease. These evolving trends emphasize the need for updated clinical protocols tailored to current histological patterns.
