Modern radiotherapy faces significant challenges when optimizing VMAT breast cancer planning for patients with extensive lymph node involvement. A recent international study led by the European Federation of Organisations for Medical Physics surveyed 25 participants to identify effective strategies. Additionally, ipsilateral lung doses varied between 12.0 and 17.3 Gy. These differences underscore how institutional priorities influence the balance between target coverage and organ sparing. Furthermore, the survey covered critical topics such as simultaneous integrated boost (SIB) prescriptions and organs at risk (OAR) prioritization.
Researchers presented five specific planning strategies that achieved high target coverage while maintaining the lowest doses to critical organs. Specifically, these methods successfully optimized the dose to the breast and lymph node volumes without compromising the safety of the heart and lungs. Because clinical goals often conflict, these proven strategies offer a roadmap for planners. Therefore, adopting these techniques can improve the consistency and quality of radiation therapy for breast cancer patients worldwide.
