Managing aphakia in eyes with significant corneal astigmatism often poses a surgical challenge, especially when capsular support is absent. Traditional methods like iris-fixated or sutured scleral-fixated lenses carry risks of suture erosion or instability. Recent research into Toric FIL-SSF IOL outcomes highlights a promising sutureless alternative for these complex cases.
A multicenter prospective study conducted in Italy recently evaluated the performance of the toric FIL-SSF lens. This specific intraocular lens (IOL) utilizes a transscleral plug-fixation mechanism. Consequently, this design eliminates the need for sutures. This simplifies the procedure and potentially reduces long-term complications. The researchers recruited fifteen aphakic eyes with regular corneal astigmatism exceeding 1.0 Diopter. Patients underwent comprehensive assessments, including manifest refraction, corneal tomography, and advanced wavefront aberrometry.
The results showed remarkable improvements across all visual metrics. Specifically, the mean uncorrected distance visual acuity (UDVA) improved significantly from 0.96 to 0.19 logMAR. Corrected distance visual acuity (CDVA) also reached a mean of 0.04 logMAR. Refractive stability was a key highlight of the study. Moreover, the mean spherical equivalent prediction error was nearly negligible at -0.12 Diopters. Furthermore, refractive astigmatism decreased from a preoperative corneal mean of 2.15 Diopters to just 0.73 Diopters postoperatively.
Stability is critical for toric lenses. Therefore, the study measured a mean IOL misalignment of only 3.33 degrees at the six-month mark. This high degree of rotational stability is essential for maintaining astigmatic correction over time. Additionally, pyramidal sensor aberrometry confirmed low total higher-order aberrations (HOAs). This ensures excellent optical quality for the patients.
Overall, this study demonstrates that the T FIL-SSF IOL provides a reliable solution for aphakic patients. It combines the benefits of sutureless fixation with precise astigmatism correction. Surgeons can expect predictable refractive outcomes and high patient satisfaction. In addition, the self-centering nature of the transscleral plugs contributes to the lens's overall success in eyes lacking capsular support.
The T FIL-SSF IOL uses a sutureless transscleral plug-fixation system. This avoids the common risks associated with sutures, such as erosion or degradation, while ensuring stable centration and rotation.
The lens significantly reduces refractive astigmatism. In the study, mean refractive astigmatism was reduced to 0.73 D, with a very low mean rotational misalignment of 3.33 degrees.
