A recent retrospective observational study conducted in Denmark provides critical insights into these real-world dynamics. The researchers analyzed data from a single general practice, focusing on 206 adults who were prescribed semaglutide for weight management between December 2022 and May 2024. The cohort was predominantly female, comprising 76% of the participants, with a mean age of 48 years. At the start of the study, the mean baseline body weight was recorded at 105 kg. This study aimed to bridge the gap between clinical trial results and routine primary care outcomes by documenting exactly how the medication was being used in a non-experimental setting. Interestingly, 40% of the patients who remained on treatment were maintained on a dose of 1.0 mg or less per week. Only 30% reached the maximum approved dose of 2.4 mg. This slower and lower titration strategy is often a pragmatic response to patient tolerability. By staying at lower doses for longer periods, clinicians can often mitigate the adverse effects that lead to discontinuation. This suggests that real-world semaglutide weight management requires a more individualized approach than the "one size fits all" titration seen in clinical trials. Practitioners are clearly prioritizing patient comfort and long-term sustainability over rapid dose escalation, which may be a more realistic strategy for chronic disease management.
Despite the slower titration and the use of lower maintenance doses, the efficacy of the medication remained impressive for those who stayed on the therapy. The study reported a relative mean body weight change of -13.6% after 12 months for those who had not discontinued the treatment. This result is particularly encouraging because it demonstrates that the clinical benefits of semaglutide are robust, even when the medication is not used exactly as prescribed in trials. Achieving a 13.6% reduction in body weight is clinically significant, as it is associated with major improvements in cardiovascular risk factors, glycemic control, and overall quality of life. For clinicians, this data reinforces the idea that semaglutide is a powerful tool in the fight against obesity, even when implemented with a flexible, patient-centered dosing strategy. It proves that real-world semaglutide weight management can yield substantial results that mirror those of high-resource clinical environments.
The findings from this Danish study offer several valuable lessons for healthcare providers managing obesity. First, practitioners should expect that real-world titration will likely be slower than what is described in the product insert. This is not necessarily a failure of treatment but rather a necessary adaptation to ensure patient tolerance. Second, because discontinuation rates are high, there is a clear need for regular monitoring and support during the first few months of therapy. Moreover, the fact that significant weight loss occurred at lower doses suggests that clinicians do not always need to push for the maximum dose if the patient is already achieving meaningful progress and maintaining good tolerability. In a country like India, where many patients may be self-funding their treatment, using the lowest effective dose could also help with long-term affordability. In clinical trials, patients benefit from intense monitoring, frequent contact with research staff, and free medication, which boosts adherence. In real-world settings, patients face practical barriers such as high costs and a lack of immediate support for side effects. Additionally, many patients in the community may stop the medication once they encounter gastrointestinal distress or reach a weight loss plateau, whereas trial participants are often encouraged to persist through these challenges.
The Danish study suggests that reaching the maximum dose of 2.4 mg is not always required for clinical success. Many patients achieved a significant weight loss of over 13% while staying on doses of 1.0 mg or 1.7 mg. Clinicians should prioritize the "lowest effective dose" that the patient can tolerate. This approach helps minimize side effects while still providing the metabolic benefits necessary for long-term health improvement and sustainable weight reduction.
Effective management of gastrointestinal side effects is crucial for improving treatment persistence. Clinicians should advise patients to eat smaller portions, avoid high-fat foods, and stay hydrated. If nausea occurs, extending the time between dose escalations or temporarily reducing the dose can be very helpful. Providing proactive guidance on over-the-counter anti-emetics and setting realistic expectations about the initial adjustment period can also prevent patients from prematurely discontinuing their real-world semaglutide weight management journey.
