Among 172 patients with endometrial intraepithelial neoplasia (EIN), 41.3% had concurrent endometrial cancer (EC) on final pathology. Age ≥50, postmenopausal status, BMI ≥35, diabetes, hypertension, and endometrial thickness ≥14 mm significantly increased EC risk. Over 60% of EC cases met Mayo criteria for lymph node dissection. These findings support early referral to gynecologic oncology for patients at higher risk of occult EC.
