For cardiovascular symptoms, most cases of tachycardia and hypertension are mild and resolve without specific pharmacological intervention. If agitation or seizures occur, benzodiazepines remain the first-line treatment to dampen the sympathomimetic response. Furthermore, continuous heart rate and blood pressure monitoring are essential until the symptoms subside. Most patients can be safely discharged after a 12-hour period of being asymptomatic. Nevertheless, clinicians should ensure that patients with intentional ingestions receive a thorough psychiatric evaluation prior to discharge to address the root cause of the overdose and prevent recurrence.
In conclusion, while pediatric methylphenidate intoxication is generally mild and self-limiting, the potential for cardiovascular and neurological complications cannot be ignored. The recent single-center study reinforces the idea that clinical monitoring is the cornerstone of effective management. Doctors must look beyond the reported dose, as it does not always predict the severity of the symptoms. Moreover, the high prevalence of intentional ingestion among females highlights a critical area for psychiatric screening and intervention. To prevent these occurrences, clinicians should emphasize the importance of secure medication storage to parents and caregivers. Education regarding the risks of sharing or misusing ADHD stimulants is also paramount for adolescent patients.
