Surgeons are increasingly adopting joint-preserving techniques for managing forefoot deformities. Specifically, MITO for Hallux Valgus (Minimally Invasive Transverse Distal Metatarsal Osteotomy) has emerged as a viable alternative to traditional joint fusion. While arthrodesis was once the standard for rheumatoid arthritis (RA) patients, modern medical management of RA now allows for more conservative surgical approaches. This study evaluated whether RA patients achieve similar success compared to those without the condition.
Researchers conducted a retrospective cohort study involving 16 RA patients and 53 non-RA patients. They utilized propensity score matching to ensure both groups had similar baseline characteristics, including age and preoperative deformity severity. Consequently, the teams assessed radiological parameters and clinical pain scores over a mean follow-up period of 38.1 months.
The study found that all radiological parameters, including the hallux valgus angle (HVA) and the first-second intermetatarsal angle (1-2 IMA), improved significantly in both groups. Furthermore, clinical outcomes such as the Visual Analog Scale (VAS) pain scores and functional health surveys showed no significant differences between the RA and non-RA cohorts. Most importantly, the complication rates remained comparable, suggesting that RA does not inherently increase the risk of failure for this specific procedure.
These findings suggest that MITO for Hallux Valgus is a safe and effective option for RA patients with stable disease. Because the procedure preserves joint mobility, patients often report higher satisfaction compared to rigid fusion techniques. However, clinicians must still select candidates carefully, ensuring that the underlying rheumatoid disease is well-controlled before proceeding with joint-preserving osteotomies. Therefore, MITO stands as a robust tool in the modern orthopedic arsenal for correcting hallux valgus across diverse patient populations.
MITO is generally suitable for RA patients who have stable, well-controlled disease and sufficient bone quality. Surgeons often prefer this joint-preserving method for mild to moderate deformities where the metatarsophalangeal joint is not severely destroyed.
The minimally invasive nature of MITO typically results in less soft tissue disruption, potentially leading to faster recovery and reduced postoperative scarring. It allows for significant radiological correction while maintaining the natural motion of the big toe joint.
