Ulcerative colitis monitoring traditionally relies on invasive procedures like colonoscopy. However, recent advancements in digital health now offer patient-reported outcome measures (PROMs) as a potential alternative. At Haukeland University Hospital, researchers evaluated the Mobile Health Index for Ulcerative Colitis (MHI-UC) to determine its effectiveness. This tool aims to detect disease activity without the immediate need for endoscopy.
The primary objective of this retrospective study was to compare the MHI-UC with the endoscopic Mayo score (e-MAYO). Researchers analyzed a Norwegian cohort of patients who completed the digital index within four weeks of their endoscopic evaluation. Consequently, the results showed a strong correlation of 0.64 between the two methods. For patients with an e-MAYO score greater than zero, the correlation increased significantly to 0.78.
Furthermore, the MHI-UC proved highly accurate in identifying moderate to severe disease activity. Patients who required clinical interventions, such as treatment changes or referrals, presented with significantly higher MHI-UC scores. Specifically, those needing intervention had an average score of 4.3, compared to just 2.3 in stable patients. This suggests that the index can effectively guide clinical decision-making by flagging patients at risk of flare-ups.
Improving
The MHI-UC is a patient-reported outcome measure used via mobile health platforms. It assesses symptoms such as stool frequency, rectal bleeding, and abdominal pain to determine disease activity levels in ulcerative colitis patients.
While MHI-UC shows a high correlation with endoscopic scores, it currently acts as a complementary tool. It helps identify patients who require urgent endoscopic assessment or treatment adjustments rather than replacing the gold-standard visualization entirely.
The study demonstrated that MHI-UC has high accuracy in detecting moderate to severe disease activity. It also reflects changes in endoscopic activity when patients undergo treatment modifications.
