Clinicians and patients generally support the therapeutic potential of medicinal plants. However, medical cannabis infection risks remain a significant concern due to the lack of uniform safety standards. While the U.S. FDA has approved three pharmaceutical-grade cannabinoids, most products available in dispensaries do not meet the same rigorous oversight. Currently, a patchwork of federal and state laws governs the industry, leaving many regulatory gaps. Consequently, these inconsistencies can lead to varying thresholds for microbial testing across different regions.
Recent reports highlight that microbial contamination is not merely a theoretical threat. Specifically, researchers have identified various bacterial and fungal pathogens on cannabis flowers, including Aspergillus and Cryptococcus. Notably, a landmark case report used genome sequencing to conclusively link a life-threatening infection from Cryptococcus neoformans directly to the cannabis consumed by a patient. Therefore, the risk of disease is evident, even if the total extent of these risks remains unknown. Furthermore, the absence of harmonized testing protocols makes it difficult for healthcare providers to ensure patient safety.
To address these challenges, clinicians must stay informed about the products their patients use. Standardized testing for microbes and toxins is essential for protecting vulnerable populations, such as oncology patients or transplant recipients. Moreover, there is an urgent need for translational research to define specific risk factors. Consequently, practitioners should advise patients to seek products from regulated sources that utilize validated safety thresholds. Harmonized regulations will ultimately bridge the gap between clinical efficacy and patient safety.
