The IMS-IMWG 2025 staging system provides a vital tool for managing newly diagnosed multiple myeloma (NDMM). This model integrates genomic data to define high-risk (HR) subsets accurately. Furthermore, clinicians now use this system to improve consistency in clinical trial reporting. Researchers recently validated these criteria in patients receiving daratumumab-based quadruplet therapy.
Specifically, a study analyzed 503 NDMM patients at Memorial Sloan Kettering Cancer Center. These patients underwent induction with anti-CD38 monoclonal antibody quadruplets. Consequently, findings showed that the new definition delineates risk better than legacy scores. Notably, the criteria identified 31% of patients as high-risk.
Although modern therapy is potent, survival differences remain significant. Interestingly, the IMS-IMWG 2025 staging did not predict early minimal-residual disease (MRD) negativity. HR and standard-risk (SR) patients reached MRD negativity at nearly identical rates. However, the genomic staging strongly predicted long-term survival outcomes.
Specifically, HR patients had a median progression-free survival (PFS) of only 2.6 years. In contrast, the median PFS for SR patients was not reached (p < 0.001). Moreover, the model predicted overall survival (OS) with high statistical significance (p = 0.003). Therefore, this staging system is essential for identifying patients who require closer monitoring or novel interventions.
