Artificial intelligence (AI) has rapidly transformed clinical practice by improving diagnostic accuracy and enhancing decision-making processes. However, its growing role in patient care raises profound ethical concerns regarding AI and patient autonomy. Specifically, healthcare providers must understand how these tools reshape the patient-provider relationship. While AI offers better information availability, it also presents risks that could undermine individual agency.
A comprehensive systematic review identifies seven overarching themes that define the ethical landscape of AI integration. Firstly, AI can support autonomy by facilitating shared decision-making. Secondly, human-centered design ensures technology respects patient values. Nevertheless, significant cognitive threats exist. For instance, algorithmic opacity often prevents patients from fully understanding the logic behind medical recommendations. Consequently, this lack of transparency can lead to "nudging" or covert control over patient preferences.
The review highlights that moral risks and social inequalities may increase if AI deployment is not ethically governed. Furthermore, clinicians face the risk of de-skilling if they rely too heavily on automated authority. To counter these threats, the healthcare community must prioritize transparency and dialogue. Ultimately, AI should serve as a tool for empowerment rather than a source of subtle control. Policies must ensure that AI and patient autonomy remain at the forefront of medical training and regulation.
In the Indian context, the shift toward digital health requires robust governance. Organizations like NITI Aayog emphasize responsible AI to protect constitutional rights. Similarly, global guidelines from the WHO stress that human agency should remain the primary focus of clinical interventions. Therefore, integrating validated measurement tools and implementation science is essential for ethical AI deployment in hospitals.
AI can complicate informed consent due to algorithmic opacity. If the provider cannot explain how a system reached a conclusion, the patient may not have enough information to make a truly autonomous choice.
Nudging occurs when an AI system subtly influences a patient's choices toward a specific outcome. This can manipulate preferences and diminish the patient\'s independent decision-making authority.
