A systematic review examining the years 2020 to 2024 reveals a significant rise in ESBL-producing E. coli across South America. This study highlights how these resistant bacteria circulate between livestock, companion animals, and meat products. Consequently, these findings provide critical insights for healthcare providers managing antimicrobial resistance (AMR) global trends. Because the transmission involves multiple hosts, a collaborative monitoring strategy is essential for public health.
The review identified the blaCTX-M gene as the most dominant resistance marker, occurring in 48.2% of the isolates. Furthermore, chickens and dogs were frequently identified as major reservoirs for these pathogens. This genetic spread demonstrates that domestic environments and food chains are deeply interlinked. Therefore, clinicians must remain vigilant about zoonotic transmission pathways in community-acquired infections. Moreover, the prevalence of these genes suggests a high selective pressure from antibiotic use in the veterinary sector.
Regarding specific bacterial lineages, researchers found that the ST10 clone was the most prevalent, appearing in 40% of the studies. Additionally, other significant clones like ST155, ST117, and ST131 were detected in various animal sources. Since these clones are often associated with human infections, their presence in meat products is a major concern. Thus, implementing a One Health approach is the best way to tackle this multidisciplinary challenge. Coordinated surveillance between human and veterinary medicine will be vital to curb future outbreaks.
