Palliative care is rapidly evolving from a clinical specialty into a critical public health priority. A new EAPC White Paper has established the primary palliative care research priorities for the next decade. This study, led by Vanderstichelen S and colleagues, utilized an international Delphi process to build a strong consensus among global experts. Consequently, these findings provide a strategic roadmap for researchers to address significant gaps in end-of-life care delivery and health policy.
The Delphi study identified 47 distinct items, with 44 receiving exceptionally high scores for appropriateness, impact, and feasibility. Most importantly, the consensus emphasizes four specific domains. First, researchers must prioritize representation and inclusion, particularly for older adults and individuals in structurally vulnerable positions. Second, there is an urgent need to improve access to care and achieve health equity across different populations. Third, the roadmap calls for enhanced social networks and critical knowledge sharing within communities. Finally, methodological development remains a core focus to ensure rigorous data collection in diverse public health settings.
Achieving equity in health and wellbeing outcomes is a central theme of these new palliative care research priorities. While palliative care services have expanded globally, many marginalized populations still face significant barriers to support. Therefore, future studies should investigate the social determinants that prevent vulnerable groups from receiving timely end-of-life care. By focusing on these items, the medical community can foster a more inclusive and effective healthcare system that supports every patient, regardless of their social or economic background.
The shift reflects a move toward a public health approach. This focus emphasizes community support, social networks, and equity rather than just hospital-based clinical interventions.
The study involved 65 international academic experts and individuals with complementary expertise from 16 different countries to ensure a diverse and comprehensive perspective.
The top domains include representation of vulnerable groups, access and equity, building critical community skills, and improving research methodologies.
