This study compared preoperative embolization with direct surgical resection in patients with skull base meningiomas to determine whether embolization improves perioperative outcomes. The meta-analysis included 896 patients across 21 studies. Preoperative embolization did not significantly reduce intraoperative blood loss, operative duration, transfusion requirements or postoperative complication rates compared with direct surgery. Meta-regression and subgroup analyses showed that these findings remained consistent across the evaluated study characteristics. The authors concluded that routine preoperative embolization has not demonstrated a clear overall advantage over direct surgery, highlighting the need for careful patient selection based on tumour vascularity, location and surgical complexity.
