Differential awakening is a phenomenon where patients experience transient neurological deficits during emergence from anesthesia. While researchers primarily observe this in neurosurgical cases involving the brain, a recent case report reveals its occurrence after peripheral nerve surgery. Recognizing this condition is vital for Indian clinicians to avoid unnecessary interventions for suspected postoperative injuries.
A 40-year-old male presented with a long history of radiculoplexus neuropathy and was scheduled for an open biopsy. Specifically, the surgeon targeted the posterior femoral cutaneous nerve near the sciatic notch. During the procedure, the surgical team performed minimal manipulation of the sciatic nerve. However, immediately after the patient emerged from anesthesia, he could not dorsiflex or plantar flex his right foot.
Consequently, the medical team closely monitored the patient. Within one hour, his motor function progressively returned to its baseline level. Therefore, the clinicians attributed this transient deficit to differential awakening rather than direct nerve injury. Pathological results later confirmed high-grade B-cell lymphoma as the underlying cause of his neuropathy.
The exact mechanisms of differential awakening remain poorly understood. However, it likely involves the asymmetric recovery of neural circuits during the transition from unconsciousness to wakefulness. In this specific case, the diseased state of the sciatic nerve may have predisposed it to a delayed functional recovery compared to the rest of the nervous system.
Furthermore, this case suggests that differential awakening might be an underrecognized cause of transient worsening in peripheral nerve surgeries. Clinicians must consider this etiology when faced with unexpected postoperative deficits that resolve quickly. Identifying this early can prevent diagnostic errors and reduce patient anxiety during the recovery phase.
