Regular screening mammography detects breast cancer at its most treatable stages, which drastically improves survival outcomes. However, many women experience delayed mammography screening due to socioeconomic barriers or lack of awareness. A recent population-based study from Taiwan suggests that even a two-year delay significantly increases the risk of advanced-stage breast cancer at the time of diagnosis.
The researchers used data from Taiwan's National Health Insurance Research Database to compare early-stage (Stages 0-II) and advanced-stage (Stages III-IV) cases. Specifically, they found that women who never underwent screening had nearly double the odds of receiving an advanced-stage diagnosis (aOR = 1.94). Consequently, the study emphasizes that maintaining a strict screening schedule is essential for early detection.
The study highlights that screening intervals exceeding two years increase the odds of advanced cancer by 42% (aOR = 1.42). If the interval exceeds four years, the risk remains significantly high. Additionally, older women and those in low-income groups showed the highest vulnerability to these delays. Furthermore, the data indicates that age-specific factors play a critical role, as women aged 40-69 saw a pronounced increase in advanced-stage diagnoses when they skipped regular intervals.
Notably, for women in India, these findings carry significant weight. Since national screening uptake remains below 1%, healthcare providers must advocate for consistent screening. Therefore, clinicians should prioritize educating patients about the dangers of postponement. Ultimately, regular screening reduces the need for aggressive treatments and improves the patient\'s quality of life.
Most clinical guidelines, including those from the Breast Imaging Society, India (BISI), recommend annual or biennial mammography for women starting at age 40, depending on individual risk factors.
Breast tumors can grow and spread during the interval between scans. Delaying a mammogram by more than two years increases the likelihood that a tumor will reach an advanced stage (III or IV) before detection.
