Bladder cancer brain metastasis represents an uncommon yet devastating clinical challenge in neuro-oncology. Historically, urothelial carcinomas spread primarily to regional lymph nodes, bones, lungs, and liver. However, prolonged survival from modern systemic therapies has unmasked an increasing incidence of intracranial dissemination. Although guidelines do not recommend routine brain magnetic resonance imaging for all asymptomatic urothelial carcinoma patients, targeted surveillance is essential for high-risk cohorts. Clinicians should maintain high clinical suspicion in patients with neuroendocrine histology, extensive visceral metastasis, or disease progression on immunotherapy. In addition, prompt administration of corticosteroids and anticonvulsants helps alleviate symptomatic peritumoral edema and prevents neurological decompensation during active treatment.
Addressing the poor prognosis of bladder cancer brain metastasis requires innovative therapeutic strategies that penetrate the blood-brain barrier. Historically, conventional cytotoxic agents demonstrated limited central nervous system efficacy due to restricted intracranial biodistribution. However, modern therapeutic advances, including novel antibody-drug conjugates and targeted small molecules, offer encouraging avenues for neuro-oncology. Emerging agents targeting Nectin-4 and Trop-2, alongside selective kinase inhibitors, warrant rigorous evaluation in central nervous system disease. Furthermore, combining stereotactic radiotherapy with targeted therapies may disrupt the blood-brain barrier and enhance local drug delivery. Future clinical trials should actively recruit patients with stable brain metastases to characterize intracranial drug responses accurately. Ultimately, translating genomic discoveries into tailored clinical trials will be indispensable for establishing effective, durable therapies for this vulnerable patient population.
