While primary tumors are common, a Cladophialophora bantiana cerebral abscess represents a rare but life-threatening possibility. This neurotropic fungus thrives in soil and plant matter. Consequently, activities like chainsaw use or gardening can lead to accidental inoculation. Despite its rarity, nearly one-third of all global cases occur in India. Therefore, medical professionals must maintain a high index of suspicion for fungal pathogens during the differential diagnosis of CNS lesions.
A recent case study describes a 36-year-old female who presented with limb weakness and an intra-axial mass. Initially, imaging suggested a tumor. However, a stereotactic biopsy revealed the presence of Cladophialophora bantiana. This fungus carries a mortality rate of up to 70%. Early identification through biopsy is vital because antifungal therapy alone often fails to penetrate the blood-brain barrier effectively.
Successful management typically requires a multi-modal approach. Surgeons aim for maximal lesional exeresis while preserving neurological function. In addition to surgery, patients receive aggressive medical therapy. Clinicians frequently use liposomal amphotericin B and voriconazole. However, achieving therapeutic voriconazole levels is difficult due to significant inter-individual variation. Moreover, medical teams must monitor patients for adverse events like renal toxicity and electrolyte imbalances.
Long-term recovery is possible with combined therapy. In this case, the patient remained disease-free for eighteen months. This outcome highlights the importance of prioritizing microbiology specimens during surgery. Furthermore, taking a detailed occupational history helps guide clinical suspicion. Early surgical debulking remains the most consistent predictor of a favorable outcome in these cases.
Patients typically present with headaches, focal neurological deficits like limb weakness, or seizures. Interestingly, many patients remain afebrile during the initial presentation.
India reports approximately 32% of all global cases. This high prevalence suggests that clinicians in the Indian subcontinent should consider this fungus when managing CNS abscesses in both immunocompetent and immunocompromised hosts.
