Cancer-associated retinopathy is a rare paraneoplastic syndrome that clinicians often link to specific immune responses against the retina. While traditional theories emphasize the role of B-cell produced antibodies, a recent case study involving chimeric antigen receptor T (CART)-cell therapy offers new insights. Specifically, a 71-year-old patient with follicular lymphoma developed rapid vision loss shortly after starting this immunotherapy. This occurrence is particularly notable because CART-cell therapy effectively depletes the B-cell population, which usually produces the antibodies associated with this condition.
The clinical presentation began only two weeks after the patient started his CART-cell treatment. He reported a sudden decrease in peripheral vision in both eyes. Consequently, ophthalmologists performed comprehensive imaging and functional tests. Fluorescein angiography demonstrated attenuated peripheral vessel filling. Furthermore, optical coherence tomography revealed the loss of the ellipsoid zone while sparing the fovea. Electroretinography results were equally striking, showing nondetectable rod responses and severely diminished cone responses. These findings pointed directly toward a diagnosis of retinal degeneration.
Diagnosis of this case relied heavily on antiretinal antibody testing. Indeed, Western blot analysis identified positive antienolase and anti-heat shock protein 60 antibodies. Immunohistochemistry also showed strong staining of the outer segments. However, the timing of these findings raises significant questions about disease mechanisms. Because CART-cell therapy depletes B cells, researchers suspect that the pathophysiology of cancer-associated retinopathy might involve more than just autoantibodies. Instead, T-cell mediated processes or pre-existing long-lived plasma cells might drive the retinal damage.
Currently, the patient remains on monthly intravenous immunoglobulin (IVIg) therapy. This treatment approach has successfully stabilized his central visual acuity. Clinicians must remain vigilant for ocular symptoms in patients receiving advanced immunotherapies. Early identification and management can help preserve vision even in complex paraneoplastic scenarios. Thus, the medical community should continue exploring the multifaceted immune interactions that trigger these rare ocular events.
Patients typically experience rapid-onset vision loss, photopsias (flashing lights), and significant defects in their peripheral or central visual fields. These symptoms often progress over weeks or months and usually affect both eyes simultaneously.
While CART-cell therapy primarily targets cancer by depleting B cells, it may trigger intense immune activation. This activation can potentially lead to cross-reactivity with retinal antigens or reveal pre-existing immune sensitivities, leading to paraneoplastic syndromes like retinopathy.
