This trial investigated whether positive affect treatment (PAT), a psychological intervention targeting reward processing, could improve clinical outcomes compared with negative affect treatment (NAT) in 98 adults with severely reduced positive affect and moderate-to-severe depression or anxiety. Participants received 15 weekly individual therapy sessions and were randomized to PAT (n = 51) or NAT (n = 47). Clinical status, assessed using positive affect, interviewer-rated anhedonia, and depression and anxiety measures, improved significantly more with PAT than NAT (effect estimate, −0.06; 95% CI, −0.11 to −0.01; P = .02; d = 0.27). The advantage remained at the 1-month follow-up (effect estimate, −0.21; 95% CI, −0.41 to −0.02; P = .04; d = 0.21). The results suggest that directly targeting reward-related processes may offer additional benefits for patients with depression, anxiety, and anhedonia.
