Immune checkpoint inhibitors (ICIs) have revolutionized the landscape of modern oncology. However, these therapies frequently trigger immune-related adverse events that challenge clinical management. One common laboratory finding is asymptomatic lipase elevation. Traditionally, clinicians have often viewed this as a benign laboratory abnormality without significant clinical consequences. In contrast, recent research reveals that these elevations may actually signal underlying pancreatic injury and subsequent volume loss.
A comprehensive study involving 1,146 patients treated with ICIs analyzed the relationship between enzyme levels and pancreatic health. The researchers utilized CT and PET scans along with pancreatic volumetry to track changes over time. They found that patients with normal lipase levels showed no evidence of injury. Conversely, those with lipase levels between one and three times the upper limit of normal (ULN) showed a 28% incidence of significant pancreatic volume loss. Most notably, in patients where the lipase level exceeded three times the ULN, 57% experienced at least 20% volume loss within one year. This confirms that asymptomatic lipase elevation is a meaningful biomarker for subclinical pancreatic atrophy.
Despite the high rate of subclinical injury, the study offers significant reassurance regarding the safety of continued immunotherapy. Researchers monitored patients who continued their ICI treatment despite having elevated lipase levels. Remarkably, none of these patients developed clinical pancreatitis or pancreatic endocrine and exocrine insufficiency. This suggests that while the pancreas may undergo structural changes, the functional reserve often remains intact enough to avoid symptomatic disease. Consequently, physicians can often maintain life-saving cancer treatments while closely monitoring these laboratory trends.
Furthermore, the data indicates that CT and PET scans are less sensitive than pancreatic volumetry for detecting early injury. While only 9% of high-lipase patients showed signs of pancreatitis on standard CT, over half demonstrated significant volume loss. Therefore, clinicians should recognize that asymptomatic lipase elevation reflects a spectrum of autoimmune pancreatic injury. Management should focus on clinical monitoring rather than immediate treatment interruption, provided the patient remains symptom-free.
Current evidence suggests that continuing therapy is generally safe if the patient remains asymptomatic. The study showed that patients who continued ICI treatment did not suffer from clinical pancreatitis or long-term pancreatic insufficiency.
A lipase level exceeding three times the upper limit of normal is a strong predictor of pancreatic injury. Approximately 57% of such patients show significant pancreatic volume loss (atrophy) on follow-up imaging, even without symptoms.
While standard CT scans often appear normal, they may miss early volume loss. Imaging is primarily useful to rule out clinical pancreatitis if the patient develops pain or other symptoms, but asymptomatic lipase elevation alone may not require intensive radiographic intervention unless levels are severely and persistently high.
