Levodopa remains the most effective symptomatic treatment for Parkinson disease, but clinicians have long debated whether starting it early accelerates later motor complications or changes disease progression. The LEAP delayed-start trial followed patients with early Parkinson disease who were assigned to start levodopa immediately or after a 40-week delay. Follow-up showed that starting levodopa earlier improved symptoms of bradykinesia, rigidity and tremor during active treatment, but the delayed-start design did not demonstrate a durable disease-modifying advantage attributable to earlier initiation.
