Pancreatic cancer remains a lethal malignancy, primarily because clinicians often diagnose it at advanced stages. One of the most common early signs is pre-diagnosis weight loss, yet its precise quantification has remained elusive until recently. A major systematic review and meta-analysis now provides robust estimates of these changes, offering a potential baseline for early detection protocols.
The study analyzed data from 25 separate studies involving over 25,000 participants. Researchers found that patients lost an average of 5.9 kg prior to their pancreatic cancer diagnosis. Similarly, the average reduction in body mass index (BMI) was 2.5 kg/m². Notably, significant heterogeneity existed between the studies. However, certain factors like diabetes status and the timing of measurements explained some of this variation.
Clinical observation of pre-diagnosis weight loss is particularly critical for patients with new-onset diabetes. In fact, when unintentional weight loss accompanies a recent diabetes diagnosis, the risk of underlying pancreatic malignancy rises significantly. Moreover, the meta-analysis suggests that weight changes occur months before other "red flag" symptoms appear. Consequently, integrating quantified weight tracking into electronic health records could help identify high-risk individuals much earlier.
Doctors should also note that weight loss timing varies among individuals. While some patients show a steady decline over a year, others experience a sharper drop closer to the diagnosis. Furthermore, cancer type and age can influence the severity of the loss. Therefore, any unexplained weight reduction in older adults requires a thorough investigation, especially if their glycemic control simultaneously worsens.
